Membership Fees
- 0-18 years old $85/month
- 18-25 years old $100/month
- Over 25 years old $150/month
- Two people $250/month
- Family of 3-5 $325/month
($20 for each additional child)
- Medicare patient $80/month
Every Membership Includes:
- Comprehensive primary care
- Preventive, acute, and chronic office visits
- Labs included are: Strep, Urinalysis, Urine pregnancy, Covid, Influenza
- Office-based, minor surgical procedures
- Women’s health
- Communication with your provider in a timely manner
- Medical advice and care coordination outside of office visits
- Lowest pricing available for laboratory
Your membership also includes wellness care, injury and sick care. Your provider will make an appropriate determination about the scope of care services offered by Nova Integrated Care on a case-by-case basis.
Some services may incur an additional fee, “itemized charges” such as pathology and labwork.
